Schedule an Appointment
To schedule an appointment please fill out our appointment request form or call during business hours. Our scheduler will take down your name, address, date of birth, phone number, and any insurance information applicable. From there, we will find a time convenient for you. Our first appointment is at 7am and our last appointment is at 6pm.
We have a 24 hour cancellation policy for appointments. Please give us 24 hours’ notice so we have the opportunity to offer your appointment to another patient. If less than 24 hours’ notice is given, you will be charged at $50 cancellation fee. If you do not show up for a scheduled appointment, you will be charged a $75 no show fee.
